Customer agrees to...

Authorization Form

I hereby authorize Mor Body Shop to make specific repairs. I understand that the full payment will be due upon release of the vehicle, including additional supplement damage charges. If customer(s) decides not to have repairs done, they will be responsible for any restocking and shipping charges.

I hereby grant Mor Body Shop employees' permission to operate the vehicle on the streets, highways, or elsewhere for the purpose of testing and/or inspecting. An express mechanics lien is hereby acknowledged on the vehicle to secure the amount of repairs thereto.

Mor Body Shop will dispose of old parts removed from the vehicle unless otherwise instructed. Mor Body Shop will not be held responsible for loss or damage to the vehicle in the case of a fire, theft, accident, or any other cause not due to negligence of Mor Body Shop employees. Mor Body Shop will not be responsible for any items left in your vehicle.

We provide 24/7 video surveillance for the protection of all vehicles and establishments.

Power of Attorney

I do hereby appoint Mor Body Shop to act as a power of attorney in fact to accept on my behalf any and all checks, drafts, or bills of any exchange, and to endorse all such checks, drafts, bills of any exchange for deposit to Mor Body Shop account for credit on my account for repairs on my vehicle.

I authorize any and all supplement charges to be made payable to Mor Body Shop.

Vehicle Release Policy

• All repairs must be paid in full prior to vehicle release.

• We do not accept personal checks.

• I understand that the estimated delivery date is not guaranteed and is subject to change.

• Storage fees may be applied.
  

Facebook and Google

• Before and after photos may be taken of the vehicle and posted online. We will try our best but cannot guarantee that license plates and other identifiers will be shown. If you do not wish to have your vehicle posted, please let us know.

We have the right to refuse service to anyone
